Medical Record Requests are handled electronically. If you are a third party organization, please complete the form labeled “REQUEST YOUR MEDICAL RECORDS” to submit a medical record request.

We value the privacy of our clients information and cannot send client records without our company’s Release of Information (ROI) form completed by the client.

Once our ROI forms and payment for records are received, records will be faxed within 10 business days. If you have any questions, please email medical.records@180-red.com.

Invoices for medical records will be sent via email. For payment, we accept credit/debit cards. Below is the pricing for medical records:
