A psychological evaluation is a comprehensive assessment that uses interviews, questionnaires, and standardized tests to better understand an individual’s mental health, emotional functioning, behavior, and cognitive abilities.

Evaluations are often requested to clarify a diagnosis, guide treatment, assess learning or attention difficulties, provide documentation for accommodations (school, workplace, or testing), or fulfill legal or medical requirements.

Forensic Assessment

Includes court-related evaluations such as anger management, alcohol and drug assessments, parenting assessments, competency evaluations, and job-related evaluations (e.g., police assessments).

Personality Assessment

Often requested by therapists for additional clinical insight or to assist primary care providers with medication management.

Required for weight-loss surgery clearance.

A deposit of $175 is required prior to the start of testing, except for personality assessments, which require a $100 deposit. All deposits will be applied toward the final cost of testing.

At this time, evaluations are offered in person at our Nashville office, located at 406 Royal Parkway. Some assessments may be eligible for virtual completion; please inquire for details.

The process usually includes:

• A clinical interview

• Review of medical, educational, and personal history

• Standardized psychological or cognitive testing

• Feedback session to review results and recommendations

Licensed psychologists or other qualified mental health professionals who are trained in administering and interpreting psychological tests.

To ensure the most accurate and comprehensive assessment, please bring the following items (if applicable):

• Photo Identification

• Medication List (or include within medical records)

• Relevant Legal Documents

• Past Medical or Psychological Records

• School Records or Reports (if applicable)

• Referral Documentation (from a physician, therapist, or other provider)

• After testing is completed, assessment results are typically available within 2–3 weeks.
• You will receive your results either in person or virtually. A copy of your results will also be provided to you.

Refunds are not issued based on the outcome of assessment results. We use nationally recognized assessments and adhere to all required testing standards. Testing will not be completed if the individual is not mentally stable to ensure the most valid and reliable results.
